NOW HIRING: Social Worker (Full-Time)
Responsibilities of a Social Worker:
- Conduct comprehensive psychosocial assessments for residents to identify their social, emotional, and psychological needs.
- Develop individualized care plans in collaboration with residents, their families, and the interdisciplinary team.
- Provide counseling and support to residents and their families to address emotional and adjustment issues.
- Collaborate with healthcare professionals, including nurses, physicians, and therapists, to ensure integrated and cohesive care.
- Advocate for residents' rights and ensure their preferences are respected in the care planning process.
- Facilitate group activities and programs to promote social interaction and community engagement.
- Coordinate and assist in discharge planning, including referrals to community resources and follow-up care.
Qualifications of a Social Worker:
- High school diploma
- LSW/LMSW preferred
- Experience with LTC/SNF
- Compassionate nature
- Serious applicants
